2025–2026 Parent Forms – Blind Brook High School

Please review the forms below. Not all forms apply to every student. Be sure to complete and return only those relevant to your child’s grade level or privileges. Each form should be submitted to the appropriate staff member listed.

Required for All Students

• BBHS Community Contract (Family Guidebook & Code of Conduct Acknowledgment Forms)

• Emergency Pick Up Authorization Form
  ➤ Return to Mrs. Velazquez in the Main Office

For Students with Schedule Flexibility

• Early Dismissal Permission Form (For Juniors & Seniors)
  ➤ Return to Mrs. Velazquez in the Main Office

• Late Arrival Permission Form (For All Students)
  ➤ Return to Mrs. Velazquez in the Main Office

For Seniors Only

• Off-Campus Lunch Permission Form
  ➤ Return to Mrs. Montemurro in the Main Office

• Senior Parking Permission Form
  ➤ Return to the Main Office
