REMINDERS

 ALL PARENTS ALL GRADES
 
We highly recommend that you SUBMIT YOUR CHILDS ANNUAL EXAM AND IMMUNIZATION RECORD EACH AND EVERY YEAR. This allows your child to remain in school, participate in physical education, fulfill necessary mandated screenings and meet immunization requirements.

In addition, ALL students who want to participate in our sports program will require a recent physical on file. The exam must be less than one year old.

ALL reports may be mailed, emailed to the nurses at MSHSnurse@blindbrook.org or faxed to 914-937-4509.

A complete Physical Examination, performed by a physician (licensed within 50 miles) within the past 12 months (please download the Health Examination Form) should be submitted within 14 days of the start of school in September. Out of state and/or out of country students have to submit a physical within 30 days of the start of school. The report should indicate the results of the scoliosis screening. The mandated vision and hearing tests are performed in our office. Your child MAY NOT participate in physical education until this is on file. Students that do not submit this will be scheduled to see our school physician.

Girls in grades 5 and 7, and boys in grade 9 who did not have a scoliosis screening noted on their health certificate should be screened in school.
   
A complete dental examination (please download the Dental Health Certificate to the right). (OPTIONAL)

All new physicals must be submitted on the mandated NY STATE physical exam form. This form is available in our offices and through the link on the right. Pediatric practices also have these. Physicals submitted on other forms or attached to the new form but left blank cannot be accepted and will be returned.

 

Flu, Conjunctivitis, Strep, & Stomach Virus:

We are aware that from time to time cases of the flu, conjunctivitis, strep, and stomach virus become present in our school. Please be aware that our custodial staff works diligently to clean the building appropriately. They have been made aware of products and solutions approved by the Environmental Protection Agency (EPA) and will be using these products to clean our facilities. Surfaces such as tables, chairs, floors, bathrooms and other common shared spaces are given extra attention.

We would also like to remind families of our school practice regarding students returning to school after such illnesses. Adhering to these practices not only helps your child, but can help prevent the spread of these illnesses in the school. 

Before returning to school:

  • Please make sure your child has not vomited for at least 24-hours of the last reported instance of vomiting.
  • Please make sure your child is “fever free” for at least 24-hours without the use of any medication.
  • Please make sure that if your child has strep throat and is on antibiotics, he/she cannot return to school before 24-hours after the first dose of antibiotics has been given.  
  • Please make sure that your child has taken medicine for bacterial conjunctivitis for at least 24-hours after diagnosis.
